چکیده

small meals (owing to distortion of the stomach and cardio-oesophageal junction); and (5) an abdominal fluid thrill but no shifting dullness, indicating a localised collection of fluid and clotted blood. The likely pathogenesis of the haemorrhage is over-distension of the stomach with tearing of a small blood vessel in the lesser omentum. The blood tracks inferiorly, stripping peritoneum from the posterior wall of the stomach, into the greater omentum. Our case shows that if the condition is promptly recognised good results may be achieved with early transfusion of adequate quantities of blood and antihaemophilic globulin and without surgical intervention.

Post-infectious Proliferative Glomerulonephritis with Monoclonal Immunoglobulin G Deposits Associated with Complement Factor H Mutation

A 55-year-old man developed rapidly progressive glomerulonephritis and nephrotic syndrome. A kidney biopsy specimen showed diffuse proliferative and crescentic glomerulonephritis with monoclonal IgG1κ, humps, and nephritis-associated plasmin receptor, indicating infection-associated proliferative glomerulonephritis with monoclonal immunoglobulin G deposits (PGNMID). Despite dialysis-dependent r...

Mixed cryoglobulinemia-associated Sjögren’s syndrome leading to spontaneous rupture of the kidney: a case report

BACKGROUND Spontaneous rupture of the kidney is uncommon and is mainly caused by renal tumors. Only a few cases are caused by vasculitis. We report here the first case of spontaneous rupture of kidney resulting from mixed cryoglobulinemia. CASE PRESENTATION A 44-year-old man presented with sudden onset of fever, acute pulmonary edema, left flank abdominal pain unassociated with trauma, and ra...

Vasculitis and rapidly progressive glomerulonephritis in the elderly.

The proportion of patients with vasculitis and rapidly progressive nephritis aged 70 years or over has risen from about 10% in the 1980s to over 30% in series reported in the 1990s. This study was undertaken to examine the presentation and outcome of such older patients. Seventeen of 56 patients (30%) who presented at two renal units were aged 70 years or over. Mean creatinine level at presenta...

Granulomatosis with polyangiitis: rapidly progressive necrotizing glomerulonephritis in a pediatric patient

Granulomatosis with polyangiitis (GPA) is associated with a broad range of clinical manifestations including renal disease. It is a systemic vasculitis that is rarely encountered in children. We present a 14-year-old girl who suffered from pharyngitis 1 week before admittance to hospital. She was admitted for macroscopic hematuria and oliguria, under the possibility of nephritic syndrome. Renal...

IgA-dominant Staphylococcus infection-associated glomerulonephritis: case reports and review of the literature

Background and objectives. The mesangial deposition of IgA is rarely described with proliferative glomerulonephritis associated with Staphylococcus infection. Recently, this association has been increasingly recognized possibly due to the increased rate of Staphylococcus infection. Design setting, participants and measurements. We report two cases of methicillin-sensitive Staphylococcus aureus ...
